ORDER:  

 Government of Andhra Pradesh in its endeavor to provide quality of life and  disease free society is making strident efforts by ensuring the required medical care  from village to State level. The disease burden due to vector borne diseases is still  alarmingly high and needs renewed focus in the strategies to be followed and  implemented in the field level. In many instances, the diseases are outcome of  unhygienic surroundings, lack of awareness, urbanization and climatic changes in  the recent past.  

2. In spite of regular annual two rounds of Indoor Residual Spraying (IRS)  distribution of long lasting impregnated mosquito nets and other mosquito breeding  control measures, the mosquito menace is not under the control and we need to  explore new strategies. In this scenario, concerted and convergent activities are to  be taken up by the Panchayat Raj, Municipal Administration, Rural water supply,  SERP, Village/Ward Secretariat department and Water Resource Departments in  collaboration with Health Department to reduce burden of vector born diseases.  

3. The life cycle of the Mosquito is 7 days in the stagnated water sources. In  order to break the life cycle of the Mosquito, the “Friday Dry day” concept is  conceived. In order to prevent the development of mosquito, all water sources  (domestic, community, irrigation and industrial) have to be cleaned once in a week.  Control of Mosquito growth is possible by community participation i.e; drying up of  all water storage appliances in all houses, removal/disposal of all empty plastic  water bottles, soft drink bottles other minor containers, flower pots at household  level. Cleaning of overhead tanks, ground level sumps, cisterns, other artificial  containers are to be taken up at community level. Maintenance of sanitation should  be taken up by removal of garbage heaps, cleaning of open/closed drainages,  chlorination of drinking water sources etc. These activities are to be monitored  closely by ANM, Village / Ward Secretariats village/ward volunteers, panchayat  secretaries, Self Help Groups, and RWS authorities in the concerned habitations.  

4. To identify and attend the source of mosquito breeding and other vectors,  the Health Medical & Family Welfare Department has developed a “Vector Control  and Hygiene app” to capture the low lying areas, garbage heaps, indoor and  outdoor mosquito breeding places for effective management of sanitation and to  control the mosquito and other vector growth. The ANMs, Village / Ward Health  Secretaries in their catchment area will visit every household on Thursday and  appraise the activities to be carried on Friday. Again on Friday, the team consisting  of ANMs, ASHAs, village/Ward Volunteers and SHG members should visit every  household of their allocated area to monitor Friday-dry day which includes  following activities:  

 (a) Team composition: Teams at Ward / Village Secretariat level shall include  ANM, ASHA, Village/ward volunteers and SHG members. The MPHA(M)/(F)  and supervisory staff have to actively monitor this activity.

 (b) Area of operation: All HD (House Dwellings) shall be targeted on every  Friday for checking of domestic and peri-domestic sources, like refrigerator,  coolers, rock grinders, unused wheel tyres, empty water/soft drink bottles,  water tanks and other possible containers as anti larval operations (ALOs) for  Mosquito larva and it’s removal.  

 (c) Community participation: Involving the community for removal of such  sources though interpersonal communication, IEC/ BCC.  

 (d) Grass root Monitoring: Uploading of outdoor and indoor breeding sources  through the Mosquito Control APP and it redressal in 24 hours have to be  taken up.  

 (e) Sanitation measures by PRRD/MAUD/Irrigation: Garbage lifting, filling up   low lying areas, Anti Larval operations, de-silting of drains must be taken up   by concerned Departments.  

 (f) Remedial measures by HMFW dept., in the areas of confirmed /suspected  cases to be taken-up as per protocol like Anti-Larval Operations(ALO),  Fogging, focal Spray and complete treatment.  

5. Coordinated activities by Panchayath Raj and Rural Development with  HMFW dept:-  

 a. Sensitization of DRDA officials, village health secretaries and village  volunteers on prevention of mosquito genic conditions.  

 b. Closing of dysfunctional wells, filling of abandoned / unwanted water  ponds.  

 c. Organizing meetings at community level for creating awareness on   prevention of Vector borne diseases, Water borne diseases and other   seasonal ailments through self help groups.  

 d. Ensure chlorination of drinking water and conducting of water quality   checks in villages.  

 e. Observation of “Friday as dryday” as a mandatory activity at all  panchayats/habitations.  

 f. conduct special sanitation drive campaigns to fill up low lying areas,  desilting of drains, removal of discarded materials  

 g. Cleaning of all open/closed drains minimum of once in a week.   h. Involving self-help groups through community mobilization for source   reduction activities to eliminate Mosquito breeding.  

 i. Make the Friday dry day as agenda for all regular meetings at all levels in   secretariat, Mandal an District level.  

 j. Issue of joint Directives(with HMFW dept) to the field level functionaries   for proper coordination.  

6. Coordinated activities by Rural Water supply (RWS) with HMFW dept:  

 a. Maintenance of uninterrupted water supply to avoid storage practices.   b. Weekly cleaning of all water sources on Friday of all schemes.   c. Proper sanitation around the community borewells and public taps.   d. Issue of joint Directives(with HMFW dept) to the field level functionaries   for proper coordination.  

7. Coordinated activities by Municipal Administration & Urban  Development:  

  

 a. Proper disposal of refuse, junk materials and solid waste to prevent  mosquito genic conditions. Imposing penalties by sanitary Inspectors/Malaria  Inspectors for abetting mosquito genic conditions.  

 b. Observation of “Friday as dryday” and to conduct special sanitation drive  campaigns to fill up low lying areas, desilting of drains, removal of discarded  materials, etc.,

 c. Constitution of Joint Team (Engineering and Health) for close monitoring of   Vector control activities at construction sites especially during rains.   d. Imparting of Training to Public Health Engineers on vector control  activities.  

 e. Sensitization of local leaders, public representatives and other stake  holders on mosquito genic conditions and their prevention and control.   f. Creating awareness regarding vector borne diseases, water borne diseases   and seasonal ailments through MEPMA staff and ward health secretaries  besides ward volunteers.  

 g. Preparation of area wise action plans for the hotspot areas where high  incidence of malaria and dengue were reported.  

 h. Provision of appropriate funds and human resources in the local bodies for   timely action in prevention and control of communicable diseases.   i. Make the Friday dry day as agenda for all regular meetings in all levels   j. Issue of joint Directives (with HMFW dept) to the field level functionaries   for proper coordination.  

8. Other line departments  

 a. SE/EE/DY EE/AE minor, medium and canal irrigation should ensure proper  de-silting of canals and branches to ensure no water stagnation happens. In  tanks and other water bodies anti larval operations have to be taken up  every Friday.  

 b. The Assistant Director Mines and Geology should identify the habitations /  breeding sites around the abandoned and working quarry lease areas and  ensure appropriate measures of mosquito control.  

9. Monitoring mechanism:  

 (a) Village/ward Secretariat level:  

 1. ANMs (HM&FW Department): Identification and updating of the  photographs of Garbage Heaps, Indoor Breeding places outdoor  breeding places, and low-lying areas through Geo-tagging.  

 2. Village/Ward / Panchayat secretary (PR&RD/MAUD Dept.) will  redress the breeding places and unsanitary places sent by the ANMs  through sanitation and environmental Health Secretaries in  coordination with community and upload photographs after work done.  

 3. ANMs (HM&FW Dept.) shall approve the action taken by the  village/Ward Secretaries after proper verification.  

 4. Upload the activities carried out in Friday-dryday in the Village/ward   Secretariat into the App.  

 (b) District level:  

 1. The DMHOs / DMO will draft village/ward level pinpoint plan in  coordination with MAUD /PR & RD and other line Departments under  the Chairpersonship of District Collector.  

 2. Make the Friday dry day as agenda for all District level regular  meetings.  

 3. Monitoring by the District Panchayat Officer (DPO) of PR&RD,  DM&HO and DMO every Monday under the Supervision of Joint  Collector(Development) about the issues raised at ANM level,  resolutions, pendency and specific penal actions for non redressal  beyond SLA.  

 4. Weekly review of all control measures taken up towards the  identified areas through Vector Control hygiene App.

 (c) State Level:  

 1. State level monitoring on weekly basis by the representative of the  PR&RD/MAUD Departments and /Health Dept. not below the rank of  Additional Directors on the issues raised at ANM level, resolutions,  pendency and specific actions for non-redressal beyond SLA.  

10. All the Spl Chief Secretaries, Principal secretaries, Secretaries to the  Government and Commissioners concerned shall take action accordingly to work in  convergence and to implement proper sanitation and control of vector borne  diseases.
